Your First Visit When you arrive at one of our office locations you will be greeted by our receptionists. If you have not already done so, you will be asked to complete several forms that will assist your doctor and our office staff in providing the care that you need.   For your convenience, we have placed a number of these forms on our site. Click here to access a list of forms you can complete online and print prior to your visit. Our physicians participate in many insurance and managed care plans. Click here to access a list of the plans in which our doctors participate, and for additional insurance information.   At the time you make your appointment, you may be asked to bring with you any x-rays, lab reports, or other studies that you have already had.  When you meet with your doctor, he or she will review your health information and perform a physical exam. If additional studies are needed, your doctor may request them.  Your doctor will talk with you about any questions that you have, and begin developing with you an individualized treatment plan. You may also meet with other personnel who will help in your care. Remember, you are an important part of your own healthcare team. If you have any questions after your visit, or any time during your treatment, contact your doctor. Good communication is crucial. We value it as much as you do.
